Sleep apnea

Sleep apnea is the term for a group of sleep disorders characterized by repeated interruptions in breathing, known as apneas, during sleep.

Breathing interruptions during sleep can be central (central sleep apnea, CSA), where during sleep the brain often fails to send a "signal" to breathe, or obstructive (obstructive sleep apnea, OSA), where breathing stops during sleep due to weakness of the throat and tongue muscles, resulting in airway obstruction. There are also mixed forms of breathing interruptions, with elements of both CSA and OSA.

Obstructive sleep apnea syndrome is the most common disorder in the spectrum of sleep-related breathing disorders, and it represents a chronically progressive disease characterized by repeated interruptions in ventilation during sleep due to complete or partial collapse of the throat. This breathing interruption is followed by a drop in blood oxygen levels and/or awakening.

Simptoms?

If OSA is not diagnosed in time, or if adequate therapy is not applied, the disease progresses and is accompanied by significant health disturbances.

OSA is often present in people with cardiovascular diseases (arterial hypertension, heart failure, atrial fibrillation, ischemic heart disease). OSA worsens chronic obstructive pulmonary disease and bronchial asthma. There is a clear connection between the presence of OSA and insulin resistance, diabetes mellitus, and metabolic syndrome. OSA is also often found in people suffering from depression, gastroesophageal reflux, and malignant diseases.

In addition to excessive daytime sleepiness, patients also experience psychological function disorders and problems in performing daily activities. There is a decline in work capacity, and on the other hand, there is an increased risk of work-related injuries and absenteeism. Drivers with OSA are more likely to be involved in and injured in traffic accidents.

The latest data comes from a 2019 study, which indicates that almost one billion people aged 30-70 worldwide currently suffer from some form of obstructive sleep apnea, of which over 400 million people (more than 45%) suffer from moderate to severe OSA that requires serious treatment.

1. What is Polysomnography (PSG, sleep study)?

  • Brain activity – brain waves (via electrodes placed on the scalp)
  • Heart activity – ECG (via electrodes placed on the chest)
  • Muscle activity controlling eye movements (via electrodes placed on the skin above the outer corner of the right eye and below the outer corner of the left eye)
  • Chin muscle activity (via electrodes placed on the chin and lower jaw)
  • Leg muscle activity – EMG (via electrodes placed on the skin of the lower leg)
  • Breathing – airflow (via sensors placed on the skin between the nose and mouth)
  • Chest and abdominal movements during breathing (via elastic bands placed around the chest and abdomen over clothing)
  • Snoring during breathing (via a microphone placed on the neck)
  • Oxygen levels in the blood (via a sensor placed on the index finger)

2. Why is there a need to monitor so many parameters simultaneously?

During sleep, our body behaves differently than when we are awake. If a sleep disorder is present, it is necessary to track whether and to what extent that disorder disrupts normal body functions such as brain activity, heart function, breathing, oxygen levels in the blood, and muscle activity. Sleep disorders not only lead to excessive daytime sleepiness but are often responsible for serious health problems such as high blood pressure, arrhythmias, coronary disease, heart failure, stroke, and metabolic disorders like sugar and fat metabolism, obesity, etc.

3. Is electrode placement painful or uncomfortable?

No. All electrodes and sensors are placed on the surface of the skin using special adhesive paste or tape. Mild temporary skin reactions like redness or irritation may occur if the person is sensitive to the paste or tape ingredients.

4. How is it possible to fall asleep and sleep with so many sensors and electrodes attached?

In most cases, patients do not have trouble falling asleep or sleeping with polysomnography sensors. All sensors are placed in a way that allows movement, turning, and even getting out of bed and walking. After a short period of adjustment, most people do not notice the sensors on their body.

5. Can sedatives or sleeping pills be used during the sleep study to help a person fall asleep?

In principle, the use of all medications and supplements on the day of the polysomnographic study should be discussed with the treating physician. It is essential to avoid alcohol, coffee, and caffeine-rich foods for at least eight hours before the sleep study.

6. Is a different sleep schedule needed the night before or on the day of the sleep study?

The person should follow their regular sleep habits, but if possible, daytime naps or sleeping should be avoided. Polysomnography evaluates the body's behavior during nighttime sleep, in line with the person’s usual habits. Napping during the day should be avoided as it may hinder falling asleep in the evening for the test.

7. Is it necessary to stop taking regular medications on the day of the study?

No. If any changes in regular medication are needed, this should be addressed in consultation with the treating physician. There is no need to stop taking medication on your own. The patient should continue taking regular medication as prescribed by the specialist at the prescribed intervals.

8. Is eating allowed before the sleep study?

Yes. The person can have dinner at their usual time, but it is necessary to avoid caffeine-rich foods and drinks at least two hours before the test.
